Out of Network Provider
As an Out of Network Provider, I will be happy to provide you with the necessary forms to seek reimbursement from your insurance company. Please contact your insurance to verify your out of network coverage.
Payment
PayPal and Zelle are accepted for payment.
Cancellation Policy
If you do not present for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
